Abramson Senior Care is looking for a Care Manager to join our team!
The Care Manager is responsible for assisting and supporting the coordination and implementation of services for clients affiliated with Abramson Senior Care. The Care Manager will represent Abramson Senior Care as a member of the interdisciplinary team and will support the Care Management team with all related activities as directed.
This role will support and advocate for seniors who are living in a variety of community and home-based settings; with focus on assessments, care planning, and ensuring the quality of life and personal needs.
The position is Full Time Monday – Friday, this is a hybrid role. A valid driver’s license and reliable transportation is required, as travel may be expected in Philadelphia and surrounding areas.
Minimum Education: Bachelor’s degree in Social Services or related healthcare field.
Minimum Experience: 1-2 years of experience and knowledge of Social Services, Geriatrics or related healthcare field.
Minimum Skills: Computer skills (Outlook, Word, Excel, etc.), knowledge of EHR systems.
